New Patient Registration

๐Ÿฅ Who can register?

We welcome new patients to the practice.

Our practice area includes Harpenden and surrounding areas. However, under NHS guidance, patients can now register with a GP practice outside their local area.

 


 

๐Ÿ“ Out of area patients

If you live outside our practice boundary, you may still be able to register as an out-of-area patient.

Please note:

  • You will not be eligible for home visits
  • We may not be able to refer you to local community services near where you live
  • You may need to attend the surgery for all appointments

Your registration will be reviewed by the practice before acceptance.

 


 

๐Ÿ“„ How to register

To register with the practice:

  • Complete a registration form online
  • Or visit reception to complete a paper form

Once submitted, your form will be reviewed by our registration team.

We may contact you if further information is required, so please ensure your contact details are correct.

You may also receive a short health questionnaire to complete after registering.

 


 

๐Ÿ“Œ Requirements

Adults

We will require:

  • Proof of address
  • Photo ID

 

Children

We will require:

  • Birth certificate
  • Immunisation record (Red Book)

We are unable to complete your registration without these documents.

If you do not have proof of ID or address, please speak to reception and we will help you.

Proof of address

  • Mortgage statement
  • Bank or building society statement (last 3 months)
  • Utility bill (last 3 months)
  • Credit card statement (last 3 months)
  • Council tax bill

Proof of ID

  • Passport
  • Driving licence
  • Residence permit
  • Birth certificate

 


 

๐Ÿ”ข Your NHS number

Your NHS number is a unique identifier used to match you to your medical records.

  • Please provide your NHS number if you know it
  • If not, we will trace it using the national system

Providing your NHS number helps ensure your records are transferred safely and quickly.

 


 

๐Ÿ”„ Re-registering or returning patients

If you are returning to the practice (for example, after travelling or as a student):

  • Please inform reception or add a note on your form
  • This allows us to link your previous records.

 


 

โณ Temporary registration

  • If you are ill while away from home or not registered with a GP, you can receive emergency treatment for up to 14 days
  • After this, you will need to register as a temporary or permanent patient
  • You can register as a temporary patient for up to 3 months while remaining registered with your usual GP
  • After 3 months, you must register as a permanent patient if you wish to continue care
  • To register, please contact the surgery
  • If we are very busy, we may not be able to accept temporary registrations, but emergency treatment will still be provided if clinically necessary
  • We cannot accept you as a temporary patient if you are already registered with a local GP practice
  • Temporary patients are not eligible for home visits
  • If you only need advice or a prescription, your usual GP may still be able to help you remotely

 


 

๐Ÿ’Š New patient prescriptions

We follow local prescribing guidelines.

This means your medication may be reviewed and, in some cases, adjusted to align with local NHS prescribing policies.

 


 

โ™ฟ Disabled patient facilities

We have facilities available to support patients with disabilities, including:

  • Accessible entrance
  • Ramp access
  • Disabled toilet

 


 

๐Ÿก Moving house

If you move outside our practice area:

  • You may need to register with a GP closer to your new address

Please contact us if you are unsure.

 


 

๐Ÿ“ Your medical records

  • Your electronic record is usually transferred within a few days
  • Full records may take several weeks to arrive

This ensures we have a complete picture of your medical history.

 


 

๐ŸŒ Non-English speakers

We can provide access to translated information and interpreter services if required.

All new adult patients are automatically sent details to join our Patient Participation Group.

The PPG:

  • Provides feedback on services
  • Helps improve patient experience
  • Runs surveys and information sessions

There is no obligation to participate, but we would encourage you to join.
